We discuss eligibility, support needs, personal goals, preferred routines, interests, and the type of home environment being sought.

02

Thoughtful Matching

Potential providers are considered based on compatibility, location, household routines, experience, accessibility, and the individual’s preferences.

03

Getting to Know Each Other

Meetings and visits help everyone become comfortable, ask questions, and determine whether the arrangement feels like a positive fit.

04

Transition and Ongoing Support

A personalized transition plan is developed, followed by regular communication, monitoring, planning, and support from Venture Living.
